Change of Name / Personal particulars including adding/deleting spouse name

Category 1: Change of Name

  1. A woman applying for change of name/surname due to marriage

  2. Divorcee / Widow applying for change of name or for deletion of spouse’s name

  3. Married applicants applying for incorporation of the spouse name

  4. Re-married applicants applying for change of name/spouse’s name

Requirements

  1. Applicants are required to fill online application form at  https://mportal.passportindia.gov.in/mission/ and carry a printout of the filled in application form.

  2. Personal presence of the applicant is required.

  3. Form-A and Form-E (e-forms. May be filled on computer only. Handwritten forms would not be accepted)

  4. Original Passport & photocopy of Passport (first two and last two pages Uploaded)

  5. Original and photocopy of valid Israeli visa.

Category 2: All other cases not covered in Category 1 including change/addition of surname and married woman wishing to revert to maiden name when not divorced or widowed

Requirements

  1. Applicants are required to fill online application form at  https://mportal.passportindia.gov.in/mission/ and carry a printout of the filled in application form.

  2. Personal presence of the applicant is required.

  3. Original Passport & photocopy of Passport (first two and last two pages uploaded)

  4. Original and photo copy of valid Israeli visa.

  5. Form-A(e-forms. May be filled on computer only. Handwritten forms would not be accepted)

  6. Gazette notification published in the Indian Union/State Official Gazette; OR Clippings of the two local (in India) newspapers OR Two public / school documents issued in the desired name OR Deed Poll as per Annexure Eduly attested by Notary public.

Category 3: To change personal particulars like change of address, appearance, place of birth, date of birth 

Requirements

  1. Applicants are required to fill online application form at  https://mportal.passportindia.gov.in/mission/ and carry a printout of the filled in application form.

  2. Personal presence of the applicant is required

  3. Original Passport & photocopy of Passport (first two and last two pages uploaded)

  4. Supporting documents for change of personal particulars as per the case

  5. Original and photo copy of valid Israeli visa.

  6. Form-A(e-forms. May be filled on computer only. Handwritten forms would not be accepted)
